The photo is credited to Jack-Johnston and shows a D28 model. The D28 was a sporty 350cc based on the crankcase of the side-valve EW, with OHV top end. Reputedly to have been designed to capitalize on the short lived craze of motorcycle football (soccer.) Note the hand change down low at the front of the frame.
