ENTRANCE FORM

To enter the Australian Douglas Centenary Rally it is necessary that you complete an Entry Form completing all the details requested. You will be deemed to have read the Conditions of Entry listed on the form and the Indemnity clause. You are required to confirm your acceptance of these by personally signing and dating the form where indicated. Failing to sign the form will render it invalid - unsigned forms will not be accepted.

The form includes a basic registration fee. This fee includes for the supply of an embroidered rally cap, scheduled barbecues and morning teas.

In addition to the basic rally fee there are a number of options that may be purchased. These are for a rally shirt with pocket and embroidered logo, additional cap or caps, a commemorative badge and tickets to the closing dinner. Please indicate your selections (being sure to nominate your shirt size) by inserting the quantity of the respective items you wish to purchase in the space provided and extend the total cost into the money column beneath the basic fee then add them to the basic fee inserting the 'Sub-Total' where indicated. If you have paid a Pre-Entry fee enter the sum of $20.00 within the paretheses below the 'Sub-Total' and subtract it to arrive at the 'Balance due and Payable' to be paid with the submission of the form - attach your money order or cheque for this amount made payable to 'CEMCC Illawarra' and mail it to...

Note that entries close on 28 February 2007. Entries received after this date will be accepted subject to availabilty and a surcharge of $A10.00 will apply. Some of the options may not be available to late entrants. Please refer to the Planning Committee for information about availability before submitting a late entry.

It is recommended that you keep a copy of the form as submitted for your records. An official receipt and dinner tickets will be forwarded attached to a further copy of the form you submitted. This will become your official pass to join the rally and must be presented on request by an official at the rally and to claim the optional items at the rally centre according to your order.

The Form

The pro forma Entry Form is provided below in two formats. The first is a .pdf (Portable Document Format) file and the second an image in .jpg format.

Portable Document Format

To use the .pdf file you will need to have Adobe Reader installed on your computer.

To install the latest version of Adobe Reader take this link and follow the instructions for downloading and installation. The application is supplied free of cost.

To download the file to your computer take this link. If you have properly installed Adobe Reader the application will be launched and the form will appear on your screen ready for printing.

To print a form from the image.

To print a form from the image below follow these instructions according to which browser you are using.

For Internet Explorer users, right click over the image. From the context menu that appears select 'Print picture...' - alternatively select 'Save picture as...' to save the form to your desktop or any other folder of your choice and print it from there as you would for any other image.

For Netscape or Mozilla Firefox users, right click over the image. From the context menu that appears select 'Save image as...' and save the form to your desktop or any other folder of your choice and print it from there as you would for any other image.

Set your printer options to print on 'A4' paper and select 'Fit to available space' or 'Borderless printing'.
